Practice schedule fault

I use the practice schedule on an iPad (iOS). This weekend I created some new items to insert into an existing routine. I change to a different app to do some practice. When I returned to safari the webpage refreshed and the previous version of the routine was reloaded. The new items were also lost. I recreated the items, they created but now say duplicate in the title. I still cannot see the original items.
I added these newer items to my practice routine and the same thing has happened again. The routine is the older one without the new items and none of the newly created items are visible,
Any ideas on the fault and how to fix.

Logged out of the site and logged back in and the last saved version is now visible. Now to see if I can run through it…
