You will find one in the note trainer app. Maybe not exactly what you’re after, but it’s a good resource.

And if you don’t fancy the Note Trainer app, Frank, then just do a Google search on ‘Guitar neck diagram’, click the ‘Images’ button, and you’ll have millions to choose from.
Print some blanks and fill it in yourself!
I am planning to start labeling them with cord note degrees and see how cords lay out over the fretboard. Exercises that are best done ones self rather than trying to learn from another’s work.

Make your own. After I did, i found I no longer needed it. I did mine in spreadsheet form. it really doesn’t matter as long as you make yourself think through how to build it.
You can always make your own. I use Guitar Scientist and I admit to being a fan-boy of the site and a subscribed user.
I created this in a couple of minutes. I’m a firm believer in the old adage, "Give a man a fish and he’ll eat for a day; teach a man to fish and he’ll eat for a lifetime.
